Large Employer looking to evaluate the effectiveness of a contracted disease management vendor on it’s members with diabetes

Project Specifics:

• Employer with 240,000 members • 12,000 members with diabetes • 4,000 members enrolled in disease management program • Disease Management Vendor contracted over the previous 12 months

Goals:

• Employer is looking to further understand the effectiveness and value of disease management interventions on their members as it relates to clinical performance

TTM’s Process:

• Develop Scientific Protocol and Sample Sizes to compare clinical measures pre and post program • Create Data Collection Tool to incorporate total management of patient and evaluation of the relative progress of the population based on the agreed upon metrics • Request Administrative data for study population and conduct random sampling per protocol • Conduct Medical Chart Abstraction • Data collection forms checked for quality assurance and scanned into database • Data Analyzed to compare before and after implementation of disease management program and statistics applied where appropriate
